Site Safety Forms is a standalone app – there are NO ONGOING SUBSCRIPTIONS FEES once you purchase this app.

Developed by a builder for the building industry. Combining years of hands on experience with my OHS Diploma safety knowledge, this no nonsense app isn’t full of colorful graphics or icons. It is a practical way to document safety and help protect the environment by reducing the amount of paper forms required onsite.

Dropbox integration also allows for completed forms to be immediately accessible for viewing by management back in the office, there is no need to go to site to review diary entries or site safety forms such as injury reports. This gives your safety manager or project manager the ability to conduct a documentation “desktop audit” of sites without leaving the office. Saving you money in time, fuel and travel costs.
Many forms have pre-defined drop down boxes for items such as weather conditions, injury type or injured body part.

The app has 9 site safety forms which include:
1 Site Diary – A prerequisite for any site or trade supervisors
2 Medical Register – A quick reference to record any worker who has a pre-existing medical condition and necessary medication
3 Site Induction Register – Record worker induction details
4 First Aid Treatment Register – Record any minor first aid injuries treated by a site first aider
5 Injury Report – A detailed record of a worker injury sustained onsite that required offsite medical treatment
6 Hazard / Incident Report – There is duty to record any hazard or incident that is reported to your site supervisor, this form not only records the hazard or incident but also corrective and preventative actions taken which could proof critical in the event of any further external investigation.
7 Record of Emergency Drill – Emergency drills are a legal and ISO requirement
8 Safety Improvement Notice – Can be used by your site supervisor and issued to a subcontractor for breaches of site safety rules or unsafe work practices.
9 Mobile Plant Register – Record details of any mobile plant brought to site, including photos.

Bonus Photo Report
While not strictly a safety form this was included after evaluating feedback during our research and development.
The photo report can be used as a weekly report or to photo document an activity such as a concrete pour or critical activity such as waterproofing.
